An Employee of the Company Has Passed Away
We’re so sorry we’ve lost a member of our Company family.
Please contact Benefits4MeEnroll at 1-844-279-7894 from 9:00 a.m. to 6:00 p.m. ET, Monday through Friday. This action will cancel the employee’s benefits and initiate the life insurance claim.
If you were enrolled as a dependent under the employee’s benefits, our Company will cover the cost of up to 3 months of COBRA continuation coverage for any of our medical, dental, and/or vision plans in which you were enrolled. You’ll also have access to the CompEAP Employee Assistance Program, with counseling that can help at this difficult time. After 3 months, you will receive a direct bill for the monthly premium payment for medical, dental, and/or vision if you choose to continue COBRA coverage.

An Employee’s Spouse, Domestic Partner, or Dependent Child Has Passed Away
The Company wants to make this difficult time as easy as possible for you. Please review the information here and reach out to the Benefits4Me Program Service Center at 1-844-279-7894 Monday through Friday, 9:00 a.m. and 6:00 p.m. ET with benefits questions. If you have lost access to your benefits due to the death of a spouse or domestic partner, you may elect coverage through the Company. Refer to the Gain or Loss of Coverage event for details.
If you’re enrolled in Company benefits today and provided coverage for the spouse, domestic partner, or child you lost, you have 31 days from the date of death to make any allowed changes to your benefits.

The Company offers paid time off from work to attend the wake and/or funeral services and address related responsibilities immediately following the death of a family member. The amount of time the policy covers depends on your employment status and relationship to the person who passed away. For complete details refer to the Bereavement Leave Policy.
