Custom Insoles with Hike MEdical

Available beginning January 1, 2027.

Don’t let foot, ankle, knee, or back pain get in the way of your day. The Company has partnered with Hike Medical to offer discounted custom orthotics—or insoles—designed to match your unique foot geometry and help reduce pressure while standing and walking—so you can move more comfortably.

Hike makes getting custom insoles simple. A five-minute foot scan is used to create your personalized insoles, which are delivered to your home in about two weeks. And with Hike’s fit guarantee, if your insoles aren’t right, they’ll be remade at no additional cost.

What you pay depends on your medical plan enrollment. Review the information below, then take your next steps towards more comfortable movement beginning January 1, 2027.

Use Your HSA

Beginning January 1, 2027, you can use your medical plan’s existing orthotics benefit toward Hike custom orthotics. The benefit has a $300 annual maximum and $2,000 lifetime maximum and is subject to your deductible and coinsurance. Hike orthotics are $120 per pair, and you and your covered dependents can each order up to two pairs per year.

What you pay for each $120 pair depends on where you are with your deductible and out-of-pocket maximum:

  • Haven’t met your deductible? You pay the full $120, which counts toward your deductible.
  • Partway through your deductible? You pay what’s left on your deductible, plus coinsurance on the rest. For example, under the Core Plan, with $100 left on your deductible: you’d pay that $100, plus 20% of the remaining $20 ($4), for $104 total. Your plan covers the last $16.
  • Met your deductible? You pay your coinsurance. For example, with 20% coinsurance, you’d pay $24 and your plan would pay $96.
  • Met your deductible, but not your out-of-pocket max? You pay your coinsurance share. For example, if you’re enrolled in the Core Plan, you’d pay 20% coinsurance, or $24, and your plan covers the remaining $96.
  • Met your out-of-pocket maximum? You pay $0 and your plan pays the full $120.

Note: If you’ve had other orthotics covered during the plan year—or have used this benefit extensively in the past—you may want to confirm with BCBSMA how much of your annual or lifetime maximum remains before ordering. Log into Myhealthtoolkitma.com or call 1-888-344-7088 to check your remaining orthotics benefit balance.
