The Company offers Care Connected, available at no cost to you, to help you navigate the complex world of healthcare.

Care Connected is a resource through Blue Cross Blue Shield of Massachusetts (BCBSMA), available to employees enrolled in a Company medical plan through BCBSMA and their family members.

BCBSMA will connect you with Care Connected when you need assistance.

In addition, a Care Manager from Care Connected may reach out to you after you receive a diagnosis or when you’re scheduled to receive certain types of care.

Care Managers will help oversee and coordinate your care by:

• Connecting you with the support you need from service advocates, registered nurses, mental health specialists, and other clinicians.
• Helping you understand your diagnosis, medications, and treatment plan.
• Working with you to prepare for a planned surgery or treatment and recovery.
• Ensuring efficient communication with and between your Healthcare providers, coordinating treatment plans, and monitoring your progress.
• Providing support and education throughout your healthcare journey. 
• Striving to enhance your overall health and satisfaction, while minimizing your costs and process inefficiencies.

Who’s Eligible

Care Connected is available to employees enrolled in a Company medical plan through Blue Cross Blue Shield of Massachusetts (BCBSMA) and their family members.

Receiving Support

While the support provided by Care Connected are available at no cost to you, any healthcare that Care Connected recommends or helps you schedule will require the cost defined by the medical plan you’re enrolled in.
